W-4 Tax Form -2008

All staff must fill out and retun a copy of this form, every year.

Staff Arrival Information Form

Please complete and return this form as soon as possible. Alternatively, you can email your arrival details to Jill.

Staff Health Form

Please have this form completed by your physician, and return it to us before your arrival at camp.

Police Background Check

Please bring this form to your local police department, have them complete it and then send it back to us. IT MUST BE STAMPED/SIGNED BY POLICE OFFICIAL.

I-9 Employment Eligibility Form

Mandatory for all NEW staff Please complete the 2nd page of this form and return it to us with the proper documentation.
